Being in a research study helps medical researchers find new drugs and treatments. Participating may allow you to receive new therapies or treatment options.
The information you provide will be made available to researchers who may contact you if they have studies that might interest you. Filling out this form does not mean you have to participate in a study. We will contact you by phone, mail, fax, or email to ask about your interest in a study and to check to see if you qualify.
You can change your information or take it out at any time by calling the LA CaTS Volunteer Research Registry at 504-988-0200. Ask to speak to the administrator of the Registry. Please call 504-988-0200 if you have any questions.
The HIPAA Privacy Rule defines Protected Health Information as individually identifiable health information that is transmitted or maintained in any form or medium. The protected health information you provide will be made available to researchers at Tulane University and Louisiana State University who may contact you if they have studies that might interest you. If you do not want to disclose your protected health information, please STOP and do NOT complete the form.
